As part of EMS week, Glen Cove Volunteer EMS has created a tool called the Letter of Life to help us help you in the event of a medical emergency. This Letter of Life can be printed out and filled out for you or your family members so that we can easily obtain information on you and your medical history so we can help you in the event you cannot give us the information yourself.
We ask that you fill out as much information as you can and place this Letter of Life in an envelope. We recommend that you keep the envelope with the Letter of life on your refrigerator labeled with your name and "Letter of Life" clearly written on it so we can quickly find it in the event of an emergency.
